Dementia in Aging Adults
Dementia or any other chronic illness is challenge of life that is very difficult to endure not only by the individual themselves but also those around them especially their family members. More than 5 million Americans live with dementia and as the population increases, this number is likely to increase placing an even greater burden on families, the long-term care system, and the economy (Jutkowitz et al., 2017). Dementia patients pay multiple costs of dementia. They not only have to pay for this emotionally but the financial impact the disease brings with itself has a vast impact on the financial stability of patients and their families. Although caring for dementia patients have positive impacts, it significantly has negative impacts as well. Caregivers report higher amounts of strain, mental and physical problems and caregiver burnout. They have also reported social isolation and financial hardships as a result of caring for a loved one with dementia (Samuels, 2023). Although the cost of dementia is high, when we think about families who have to take time out from their daily lives to care for the patients, they are not only missing out on the positive pleasures of their lives, but they are also unable to use that time to work or earn income. Along with that caregivers can also get affected by the psychological well-being of dementia patients and in the long rum be victims of anxiety and depression. When it comes to the economic cost of dementia, it does not get any better. Caregivers of patients with dementia get worn out easily or they are unable to care for these patients due to their work so many patients end up going into long-term care. In a study when dementia cost is discussed, researchers found that a person with dementia receives greater than $250,000 worth of care. Another study found that those with dementia receive greater than $56,000 in additional care in any given year compared to those without dementia. In the same study, they analyzed the informal caregiving rate, which was about $19.71, and the value of formal caregiving was $23/hour. They found that total and net cost of dementia over a lifetime of dementia was $321,780 and $184,500, respectively (Jutkowitz et al., 2017). As a service provider for aging adults, the financial impact of my ability to care for and run my department is vast because these are payments not always covered by health insurance. Due to the payments not being made by patients on time, it is difficult to provide service without the financial burden of running a business smoothly. If the patient is covered through insurance or they have means of fulfilling their financial obligations, no such burden should impact one from giving proper care to their patients and residents.

- From the readings, identify two factors associated with healthcare spending that must be addressed. Why did you choose those two factors?
According to Dielman et al. (2017), health care spending has increased by 17% in the 20 -year span of 1995-2015. It is reported that there are five fundamental factors involved in this growth of spending: population size, population age structure, disease prevalence, service usage, and service prices and intensity (Dielman et al., 2017). Population size, structure, disease prevalence, and service usage cannot be adjusted, so I will address service prices and intensity. According to Dielman et al.(2017), price and intensity were the largest contributors to health care spending. Most of this cost arose from inpatient settings, namely for spinal care of the back and neck, and also on drugs for diabetes. The high cost of prescription drugs strains healthcare budgets and limits the funding of other areas that would benefit the public (Rajkumar, 2020).
- What is a possible solution to efficiently and effectively manage the care of the chronically ill?
The financial situation of caring for a chronically ill person can be overwhelming. Those who suffer with dementia, also may concomitantly have serious other medical conditions which need special care and attention. The types of services that these individuals need are not part of the services that are covered by ordinary health insurance (Coe et al., 2021). Medical care in the United States is pointed directly toward acute care and this presents serious financial strains on families and on the states, should the individual be uninsured. In this limitation of coverage comes an obstacle for the development of evidence-based dementia care models ( Coe et al., 2021). The Medicare Advantage program , The Program for All Inclusive Care for the Elderly (PACE), are two financial support structures for coverage of the cost of the care of the chronically ill that would provide the needed care to those suffering with dementia (Coe, 2017).
- Describe the current burden of chronic mental illness on our healthcare system.
Multiple studies have shown that the US healthcare system is responsible for $219 billion dollars annually to support care for bipolar disease I and II, of which 3.1% of the population are a part of. (Bessonova et al., 2020). Most of these costs are related to lost work days, unemployment, and caregiving (Bessonova et al., 2020). The high direct costs are reported to be related to acute care. Of those using a greater amount of health-care resource utilization ( HCRU) were patients who did not adhere to their pharmaceutical program and those with comorbid conditions (Bessonova et al., 2020). This population is just a small example of the financial strain on the healthcare system in terms of mental illness.
- What factors related to mental health disparities and cost did you find surprising?
I found the staggering dollar amount to be the most surprising; $219 billion dollars to support bipolar disease I and II alone. Surely, that total is higher given all the other possible mental illness that were not accounted for in that particular article. What I had not considered, was the outsourcing of money for lost work days and unemployment due to the illness, as well as the cost it entails to supplement those caretakers who then must take days off, or take FMLA to care for these individuals.
- Discuss how these challenges would impact you as an aging service provider and leader in a long-term care organization.
The largest challenge I see as impacting the aging service provider in a long term care setting would be financial. Many of the individuals in long term care suffer from dementia and require much care in the activities of daily life. This puts an added demand on the number of personnel needed to deliver these services. In addition, the individuals suffering from dementia may also have serious health issues that cannot be served in a long term care facility due to lack of insurance coverage for these services. The struggle for these organizations is financial from the two directions presented. The assistance of more government insurance programs for individuals with dementia and concomitant illness would aid in these individuals finding a place for quality care. These individuals, when cared for at home, are brought over and over to the hospital for treatment that is not available elsewhere.

Hi Shagufta and thank you, as always for a very insightful post.
The challenges you mentioned include the physical and financial strain on caretakers, as they may have to stay home to care for the individual, and also the financial compromise for those who pay caretakers to care for their loved ones. In terms of policy solutions for such issues, many other countries have weighed in with their plans for caring for the elderly with long term car insurance provided by the government. Looking to China, one of the most populated placed in the world, there is an article dealing with this topic. Chen and XU (2020), conducted a review of the evolution of the public long term care insurance in different countries and examined the influence and challenge of this implementation. With the population exploding wordwide, the elderly population has increased exponentially. China is feeling the daunting occurrence of and increase in disabled elderly. Looking at Germany and other countries, it is reported that the crisis is alleviated by promoting public Long Term Care Insurance (Chen & Xu, 2020).What this involves is a significant notice by the government to the cost of caring for this population and the substantial burden it places on individuals and families (Chen & Xu, 2020).
Once again, we touch on the topic of Americans having to spend down their savings in order to qualify for long term care insurance provided by the government. I believe, as Americans, we deserve to have affordable and comprehensive solutions to long term care needs without having to spend down our asset for eligibility. This topic has floated around the round table for years, perhaps now, with aging baby boomers, there may be focused attention on a solution.
