My organization utilizes a combination of capital budgeting, statistical projection, revenue budgeting, and operating budgets, that I am aware of. I didn’t give much thought into the capital budget until I moved into the clinic setting, and the fiscal year was ending. We were asked to submit any capital spending requests. This is where I learned that the capital budget was the equipment and infrastructure. We submitted for new waiting room furniture, and for an office enclosure with a door for a new provider that was starting. We are always looking at the statistical projections with each provider’s patient panel, how many new patients they can see each week, and how they can grow their panel for the next year. Revenue and operating budgets are linked in a way. Revenue is how much money is brought into the organization, and the operational budget is how much money it costs to run the organization (Zhang & Bohlen, 2023). This is always interesting to me because the revenue brought in always seems much greater than the cost. The organization seems to try to do more with less, especially when it comes to staffing. When I started in the clinic setting, there was a staffing matrix of 1.7 medical assistants per 1.0 fte’d provider and 1 RN per 4 providers total, regardless of fte. Now that the fiscal year has passed, the organization has changed the matrix to not as specific. Now, the staffing isn’t based on provider fte, but the provider’s panel size. After doing the math, it seems to be 1.2 medical assistants to each provider and 1 RN per 6 providers. While I understand that there needs to be some sort of budgetary regulation, overworking staff will only cause turnover, which can have even more of a negative effect on budgets.
