In my current position, the way I would apply CQI is to decrease the number of hypertension crisis at the outpatient clinic. Many of the patients does not know the type of anti hypertensive medications that they are currently taking. Hence, this leads to non-compliance. When they follow up with their primary providers, many patients have extremely high blood pressure and are symptomatic. Unfortunately, those patients are transferred to the ED and be admitted to inpatient. These type of patients could benefit from nurse clinic, where nurses can educate them about the medications, how to check their blood pressure, and risks of hypertension. Patient education and engagement is more important in primary care setting compare to in patient because providers can patient better understand their condition and make better choices for their health (AHQR, 2023). In primary care setting, nurses can develop better rapport and focus on patient’s health beliefs and preferences when guiding patients to improve their health. Continuous quality improvement (CQI) occurs when an organization makes appropriate changes towards improvement in processes, safety and patient outcomes. The goals of CQI include but are not limited to improvement in operations, outcomes, work environment or regulatory compliance. Specific goals for CQI can be developed through a process called benchmarking; in this process performance is compared to similar institutions or core measures. Each goal should have specific measurable outcomes that can be tracked to help determine improvement vs deterioration and appropriate interventions can be applied. It is in this manor that evidence-based practices are put into place and necessary changes are made to ensure continued improved outcomes for our patients.
An example of applying CQI in my previous position consisted of monitoring and providing appropriate interventions for central line associated blood stream infections (CLABSIs). As chair of my unit’s practice council, we chose a specific quality indicator or area of improvement that our unit was struggling with and came up with interventions that could help improve the number of CLABSIs our patients were experiencing, which in turn, would also improve our quality indicator scores and patient outcomes. Our practice council team developed a certain process that was to be utilized when caring for any and all patients admitted with a central line; if a patient would develop a CLABSI during admission, we would then have the RN caring for the patient complete a questionnaire to determine if the CLABSI bundle was followed, the patient chart was also reviewed to ensure for policy compliance. We would then review that data and make appropriate policy changes and update standards of care for our unit based on the trends we were noticing on our unit.
