If at the end of the education they still do not agree respect the wishes of the patient and family, and take care of the patient with other methods that they wish to be taken care of. P: Practice Question, E: Collection of Evidence, and T: Translation of evidence into practice. This model is one of the easiest for new nurses to understand. This model breaks each section of PET into steps for for EBP (John Hopkins Medicine,2017). The strength of gathering strong evidence gives consistent results and determines if the evidence is supported to put into nursing practice.
The importance of integrating both evidence based practice and patient preferences, is the patients outcome. The patient needs for the health care team to be vested in their care to build integrity, trust and better patient outcomes. There is an abundance of definitions of evidence-based practice (EBP). Fortunately, most of them say essentially the same thing. According to an article written in American Speech-Language-Hearing Association (2004), the goal of EBP is the integration of (a) clinical expertise/expert opinion, (b) external scientific evidence, and (c) client/patient/caregiver values to provide high-quality services reflecting the interests, values, needs, and choices of the individuals we serve.
Because EBP is client/patient/family centered, the nurse’s task is to interpret best current evidence from systematic research in relation to an individual client/patient, including that individual’s preferences, environment, culture, and values regarding health and well-being. Ultimately, the goal of EBP is providing optimal clinical service to that client/patient on an individual basis. Because EBP is a continuing process, it is a dynamic integration of ever-evolving clinical expertise and external evidence in day-to-day practice.
When EBP nurses practice and family do not mix, then the patient and his family have the final word. Once the patient and/or family have been properly educated and the benefits of EBP have been thoroughly discussed, the final decision will always rest with the patient/family.

The systematic review from the list of approved reviews for my capstone project that I chose was Hospital Readmissions with the focus on : Are most discharge interventions effective enough to prevent the risk of early readmissions?
I work on a medical surgical orthopedic floor, I constantly see the same patients readmitted frequently, so this makes me very interested in the effectiveness of discharge intervention for the patient and family, do they really understand how to take care of themselves at home, are they using the resources, such as case management, social services, outside community resources, that were given to them, are they going to their follow up appointments, taking medication as prescribed. Overall this it’s effects unnecessary cost and the economy. Many patients are not able to afford the resources, follow ups, medication, so how do we help them “gain access and enact the care” that is needed
