Sample Answer for NURS 6050 POLITICS AND THE PATIENT PROTECTION AND AFFORDABLE CARE ACT
Politics has a variant of definitions; the definition of politics by Milstead and Short (2019) is “the process of influencing the allocation of scarce resources” (p. 38). During election time, politicians support or oppose legislation on economic and social issues using various strategies to get the political support of voters. President Obama implemented the Affordable Care Act (ACA) in 2010 to provide Americans with improved and expanded access to healthcare coverage through Medicaid and private insurance companies. Since the enactment of the ACA, the Republican and Democratic parties have had diverse stances on the matter. It continues to be a topic during the electoral and re-electoral process to gain public support, as adequate coverage for health insurance still needs to be improved to meet the demand.
President Trump attempted to repeal and replace the ACA in 2016 with the American Health Care Act (AHCA), which created a more significant divide between the Democratic and Republican parties due to eliminating Essential Health Benefits (EHB) (Willison & Singer, 2017). As a result, this hurt the momentum of support for repealing the ACA due to the loss of EHB and a sufficient replacement program. At the same time, the Biden administration has continued to build on the framework of the ACA that was put into place in 2010 by working on expanding language access to promote enrollment. In 2023, the Biden administration invested nearly 100 million into the Navigator grand fund to help assist with coverage enrollment and promote marketing (Keith, 2021). These changes allow for the protection and expansion of healthcare coverage for minorities with difficulties with English as their primary language, allowing for greater health literacy. Reviewing the actions of President Biden and Trump and see how different the two political parties’ actions are on the ACA. The Biden administration continues expanding coverage, and the Trump administration pushed for repeal and replacement.
The citizens of the United States play an essential role in how legislative leaders form policies. As politicians gather support, many strategies are in progress, almost as if they are playing a game by aligning or opposing policies to obtain public votes (Milstead & Short, 2019). A policy feedback loop is measurable to evaluate the influences participation and affects political attitudes and support toward candidates (Mettler et al., 2022). An example regarding the ACA is that those who gained from the positive benefits of the ACA are more likely to participate in voting. They risk losing their coverage, risking the candidate’s likelihood of being elected. With the upcoming 2024 election, the ACA will continue to play a significant role in each candidate’s campaign.
Attempts to repeal and replace Obamacare (the Affordable Care Act) and the failure of policymakers to adopt the American Health Care Act, which would benefit future voters, all contribute to the formulation of healthcare policy politics. The Patient Protection and Affordable Care Act (ACA) of 2010 was a progressive plan; It made a concerted effort, maybe for the first time, to address social justice issues in American healthcare. However, the policy’s implementation was plagued by political overtones, as the legislation was openly unfriendly to Republican members. When the Trump administration took office, they prioritized repealing and replacing the Affordable Care Act. ACA supporters assert that the president’s actions mirror his words, that he is deliberately “sabotaging
Links to an external site.” enforcement of the ACA(Jost,2023,June12.thecommonwwealthfund.org). As soon as legislators understood that the latest measures to “repeal and replace” would affect the average American voter, support for the Republican efforts quickly eroded. Many Republican lawmakers began to realize that maintaining their support for repealing and replacing the Affordable Care Act (ACA, also known as Obamacare) was detrimental to their chances of being re-elected.
Repealing and replacing
In May 2017, the House of Representatives enacted the American Health Care Act (AHCA) as the first step in repealing and replacing the Affordable Care Act. The AHCA was intended to be the vehicle through which the Trump administration and Republican lawmakers would replace Obamacare. However, it quickly became apparent that 24 million Americans would lose health care coverage under the AHCA.
The American Health Care Act proposed replacing income-based tax credits with age-based ones. Many Americans’ healthcare costs would have been higher just because of this. After 2026, premiums will be lowered by 20% thanks to a new Patient and State Stabilization Fund. Americans with preexisting medical issues will see their premiums rise because of this. In addition, the ACA’s replacement bill will allow states to temporarily deviate from some of the ACA’s regulations under certain circumstances. This will also increase costs for Americans with preexisting conditions to maintain health insurance coverage.
Re-election, as in efforts to repeal and replace the ACA
Soon after, a cost-benefit study by the legislators, including Republicans, showed them that the far-reaching negative features of ‘repeal and replace’ risked not getting them re-elected. As mentioned, the proposed plan will deny many Americans access to adequate medical care. If the Republican effort to repeal the Affordable Care Act and make severe changes to Medicaid is successful Major gains in economic security would be lost if the ACA were repealed, particularly the extension of Medicaid to low-income people. One in five Americans had coverage through Medicare and Medicaid in 2020, totaling 12.5 million. I’m sure the effects of this many people losing their health insurance will have some domino effect on reelection.

Sample Answer 2 for NURS 6050 POLITICS AND THE PATIENT PROTECTION AND AFFORDABLE CARE ACT
Despite the Affordable Care Act (ACA) having helped insure more 20 million people since its adoption, the legislation has faced strong political opposition since its adoption. There has been repeated calls from both political arenas to repeal the law and replace it with alternative reforms or modify the act to address other goals. The first attempt to replace the ACA was attempted by the republicans and the white house which aimed at bringing the American Health Care Act (AHCA) but failed. Nonetheless, the republicans have ever since resurrected the bill and passed the AHCA although they had to make amendments which allows states the option to waive Essential Health Benefits (EHB) requirements.
The ultimate outcome of repealing or replacing the Affordable Care Act remains uncertain. However, it remains clear that the republicans will continue to persist on repeal and replace efforts. The Trump administration is focused on unifying the republicans agenda by standing by the full repeal and replacement of the ACA.

Sample Answer 3 for NURS 6050 POLITICS AND THE PATIENT PROTECTION AND AFFORDABLE CARE ACT
The Patient Protection and Affordable Care Act of 2010 (ACA) is a revolutionary policy. This Act wanted to address social injustices in the American healthcare system. However, this policy has met stiff opposition from Republican legislators who openly opposed it. When Donald Trump came to power, repealing ACA was one of his number one priorities. They, however, did not get the requisite number to repeal ACA since other legislators felt like many Americans would be affected. Many Republican lawmakers started seeing that they may not get reelected if they continued to support the repeal of ACA.
In May 2017, the House of Representatives passed the American Health Care Act (Jost, 2017). This was the first attempt at repealing ACA. The Trump administration wanted to use this act to repeal ACA. However, this Act would make millions of Americans lose health insurance coverage. AHCA proposed that tax credit be based on age, which would increase healthcare costs. This Act also created the Patient and State Stability Fund, which would reduce premiums by 25% after 2026 (Jost, 2017). This would increase healthcare costs for Americans with preexisting comorbid conditions.
A cost-benefit analysis by Republican lawmakers revealed that they would not get reelected because of the negative effects of repealing. Repealing ACA would lead to millions of Americans losing insurance coverage (Straw & Aron-Dine, 2020). This was the main reason for implementing ACA. ACA aimed to increase insurance coverage to millions of Americans and reduce healthcare costs (Sommers, 2020). Due to reelection purposes, the Senate Majority Leader McConnell found it hard to garner support for AHCA. To protect their political interest, the US senators came up with another proposal known as the Better Care Reconciliation Act of 2017 (Smith & Gibbs, 2017).
Members of Congress and Senators are elected leaders who must be voted for by the public. Due to this, their positions on policies such as Obamacare are influenced by how their electorates feel. Because of this, their interest is reelection and not ACA.

Sasha,
The last statement of ”….their interest is re-election and not ACA.” This statement highlights the level of politics within politics. Interest groups spend resources on trying to remove legislators who do not share in their intentions. Americans would be better served by policy-motivated committees that dissect and evaluate the benefit of a bill before allowing it to be considered (Lorenz, 2020). Everything boils down to the dollar. I understand health care and politics are as much of a business as anything else. However, it is unfortunate the system is overly saturated with misguided interests, and how does this benefit me. Personal experiences weigh heavily on attitudes toward the government and health care policies within contemporary societies (Larson, 2020). Lobbyists aid in providing and identifying agendas; these “agenda setters” would be more likely to perform in the public’s best interest if a policy-motivated committee was the influence behind new bills (Lorenz, 2020).

Hello Sasha,
Thank you for your response to the discussion. The way you ended your post really stands out and is a little unsettling. It is unfortunate that there are politicians who’s main goal is to keep their position or maintain their image for re-election. I would hope that the majority of politicians stand by their ethics and vote based off their true feelings and advocate for the populations they represent. I would like to see improvements continue to be made in relation to health care without driving the costs of care up. It is obvious that politicians want to make changes, for example, the Better Care Reconciliation Act was created in order to make changes but not to be as controversial (Smith & Gibbs, 2017).

Have you ever heard the saying; you have to be in it to win it? This saying can summarize the actions one must take to play the tricky game of politics. Whatever the issue of concern at hand is, one must be strategic in getting their views across to land in the hands of the right political officials. Like moving a token across a game board, it can take a slow, one move at a time to prevail. We as nurses hold specialized expertise views on a vast amount of healthcare concerns and topics, making it that much more imperative to get involved in the political process. We must use our knowledge to effect change for the overall health and wellness of the public. The chain process of passing along an item of agenda or aiding in creating a bill can be cumbersome but extremely rewarding if achieved. All moves are links in the chain and must be mapped out to the best of our abilities to obtain forward momentum. The first step is by doing your research on the topic at hand. What is the current issue? Why do we seek change? How do we achieve change? What outcomes can be expected by this change? Also, knowing which individual legislator deals with the specific category topic at hand is monumental because this is the first person you must convince of the issues importance, it starts with them. Be prepared to be familiar and informed of that person’s views and standpoints on the topic. Possibly, finding similar ground to speak on and giving detailed examples can get their attention. Also, making the topic personable to them can spark that interest needed to push along the bill or need of change being presented. Though not easily admitted, almost all politicians end goal is to be reelected each term. For this reason, passing new bills can be either positively or negatively impacted. A representative may not choose to vote or push a topic of interest through to the next level due to fear of political repercussion. Vice versa, they may choose to support a topic at hand if it is something that they believe may influence a spike in votes for their position. Political representatives can be influenced by the masses and current life events making news, knowing how to position your issue of concern to reach them in a way that may aide in their re-election may give you common ground to stand on. Making what is important to you, be important to them. Who is elected to office, whether at local, state, or federal levels will have a direct impact on public health. Public health constitutes more than just healthcare, it involved the persons overall health status which can be positively or negatively impacted by things such as housing, education, healthcare, and livable wages. (Beilenson, 2018).
In 2010, the 44th U.S. President Barack Obama signed into law the Affordable Care Act, also known as Obamacare. “The law provides numerous rights and protections that make health coverage more fair and easy to understand, along with subsidies (through “premium tax credits” and “cost-sharing reductions”) to make it more affordable. The law also expands the Medicaid program to cover more people with low incomes.” (U.S. Centers for Medicare & Medicaid Services, 2020). When considering repealing or replacing the ACA, we must look at who promotes it and who opposes it and what effect it can have on the overall public. Democratic parties “Support universal healthcare; strong support of government involvement in healthcare, including Medicare and MedicaidLinks to an external site.. Generally support Obamacare.”, whereas Republican parties “oppose Obamacare provisions like (1) requirement for individuals to buy health insurance or pay a fine, (2) required coverage of contraceptives.”. (Diffen, 2020). Those individuals that benefit from the Affordable care act are the middle class and lower-class society, whereas in comparison those whom oppose the ACA tend to be the upper class who will see a tax increase. According to the American Progress website, if we repeal the Affordable Care Act we can expect that “More than 20 millionLinks to an external site. people would lose their health coverage, and more than 135 millionLinks to an external site. people would lose protections for their preexisting conditions, including millions of COVID-19 survivors. Repealing the ACA in the middle of an unprecedented pandemic would create chaos across the entire health care system; weaken the country’s public health and economy recovery; and rip affordable health care coverage from millions of people at a time when access to health care services is absolutely essential.” (Cusick Vice, et al, 2021). Lastly, knowing which side of the fence you stand on politically, having a strong evidence based supported plan ready to be initiated and knowing your elected officials’ standpoints and views on the subject matter is paramount to swaying the legislator’s opinion and when promoting your concerns.

The purpose of this bill is to provide access to reproductive health care services for all persons, regardless of their financial standing. Ms. Schrier is the bill’s primary sponsor, and her Democratic and Republican colleagues, Mr. Bera, Ms. Caraveo, and Mr. Ruiz, are all co-sponsors. Abortion, contraception, in vitro fertilization, comprehensive treatment, counseling, and education are all included in the scope of the bill. Part 2: Legislation Testimony/Advocacy Statement
Testimony in Support of H.R. 2907
It is a privilege for me to be here today to provide my full support to H.R. 2907, often known as the “Let Doctors Provide Reproductive Health Care Act.” During my testimony, I will mostly be pushing for the passage of this vital piece of legislation, discussing the effect of socioeconomic determinants of health, and reacting to any objections that may arise.
Advocating For H.R. 2907
The passage of H.R. 2907 is a major victory for the cause of ensuring everyone has the fundamental right to use reproductive healthcare services. It acknowledges that access to reproductive health care is not only essential to an individual’s well-being, but also a basic human right. Individuals will be able to make educated choices about their reproductive health because of this legislation’s guarantee of the unfettered availability of legal reproductive health care services by medical professionals. Furthermore, it sets up systems for legal protection and safety nets for both providers and patients.
Impact of Social Determinants
Access to and satisfaction with reproductive health care services are strongly influenced by social determinants of health such as socioeconomic status, age, level of education, and gender. For instance, those with lesser incomes may not be able to purchase these services, resulting in postponed or nonexistent treatment. Access to reproductive health care is especially difficult for young people because of legal and financial restrictions, as well as stigma. The capacity of young people to make educated decisions is hampered by a lack of comprehensive sexual education in schools.
Misinformation and a lack of understanding about available reproductive health alternatives may also result from gaps in education. The healthcare system is only one place where women and non-binary people face prejudice and uneven treatment because of their gender. In light of these inequalities, H.R. 2907 aims to eliminate barriers to care, increase access to education, and guarantee that all people have equitable access to reproductive health services.
Addressing Opposition
H.R. 2907’s detractors have raised concerns that the bill violates citizens’ right to freedom of thought and religion. Recognizing and appreciating the variety of perspectives out there is essential, as is stressing that no one is being forced to offer or seek reproductive health care services against their will by this legislation. Instead, it protects people’s freedom of choice and ensures that medical practitioners may practice to the best of their abilities. To address issues, it is critical to highlight the public health advantages of reproductive health care as well as the value of individual choice.
