Re: Topic 4 DQ 1
Hi Dr. Terry. A key takeaway for me is the involvement of the many initiatives that address social determinants of health within and outside the healthcare system. Outside the health care system, initiatives seek to shape policies and practices in non-health sectors that promote health and health equity. Within the health care system, there are multi-payer federal and state initiatives, and Medicaid-specific initiatives focused on addressing social needs. These include models under the Center for Medicare and Medicaid Innovation, the Medicaid delivery system, and payment reform initiatives and options under Medicaid. Managed care plans and providers also identify and address social needs (Anna Spencer, 2016). For example, 19 states required Medicaid-managed care plans to screen for and provide referrals for social needs. A recent survey of Medicaid-managed care plans found that almost all (91%) responding programs reported activities to address social determinants of health (Spencer A. et al., 2016). In addition to the growing movement to incorporate health impact/outcome considerations into non-health policy areas, there are also emerging efforts to address non-medical, social determinants of health within the context of the healthcare delivery system (Bachrach, D. et al., 2018). These include multi-payer federal and state initiatives, Medicaid initiatives led by states or by health plans, and provider-level activities focused on identifying and addressing their patients’ non-medical and social needs. According to Bacharach et al. (2018), within Medicaid, the growing focus on social determinants of health raises new questions about the appropriate role Medicaid should play in addressing non-medical determinants of health and how to incentivize and engage Medicaid MCOs in addressing social determinants of health.

Sample Answer 2 for DNP 825 Evaluate how social factors, such as personal or professional values, influence both individual and population health
Individual and population health can be heavily influenced by various social factors. Social factors can be closely linked with health disparities and limit the overall quality of care for individual or groups of people. One social factor that impacts health is education. Research shows that education can be linked to adherence to preventative care (Raghupathi & Raghupathi, 2020). The lack of health education can limit health knowledge which could lead to poor health choices. Being well-informed can promote better lifestyle choices leading to better health. It is also known that obtaining a higher level of education can lead to better paying jobs. This leads to another social factor, income. Higher income levels can allow groups of people to have better access to health. Having lower income can prevent someone from having affordable health insurance which could impact how often they may see a doctor. It can also prevent people from being able to afford healthier food options and decrease the time they have for exercise. All these things can directly affect one’s health.
From a worldview perspective it is my belief that everyone should have equal access to quality care. No matter the circumstances no one deserves to have limited access to care because of social factors. While some social factors are determined by individual choices others are influenced by political climate or other economical factors. For instance, during COVID many people lost their jobs. McCartney et al., (2020) research confirmed that insecurity and unemployment is negative for health, in particular mental health. When people have poorly managed mental health, they are at increased risk for physical health problems. Giving people equal access to healthcare, in spite of social factors, will lead to healthier population that requires less medical care.

Sample Answer 3 for DNP 825 Evaluate how social factors, such as personal or professional values, influence both individual and population health
The Covid-19 outbreak made countries implement measures to reduce the number of infections. These measures would only work with public support. The importance of worldviews and public trust in the implemented measures are based on the drivers of social trust. The widespread prevalence of Covid-19 has resulted in severe economic losses and restrictions on freedom, and this crisis provides an opportunity to examine the factors influencing people’s acceptance of measures aimed at decreasing the number of infected individuals (Siegrist, 2021).
An important factor for public acceptance measures is trust, which may affect how people perceive risks and accept measures. Social trust during the pandemic implies that the government provides unbiased information and that the risks are not excessive. The different measures were positively correlated with risks and acceptance of implemented measures. General beliefs and value orientations on cultural worldviews were considered factors influencing people’s risk perceptions and fears, which can be an important driver influencing their behavior in this situation (Siegrist, 2021). Those individuals who perceive more risks associated with Covid-19 show more acceptance of the measures in place than those who take fewer risks.
People in the community might have a different view of the nurses that provide care for them, and this lack of understanding of alternative worldviews contributes to poorer health outcomes. As we work with different groups and individuals, we have to find ways to ensure a more embracing, culturally responsive healthcare environment that values and respects the belief of others.
