PICOT and the Patient Population
PICOT: Among adult patients with type 2 diabetes in a nursing home, will the translation of Nelson et al’s research on the use of telehealth text messages compared to the current practice improve HbA1c levels in 12 weeks?
As highlighted in the PICOT, the selected patient population is adults with type 2 diabetes in a nursing home. Commonly, nursing homes have many adults with or at risk of diabetes due to advanced age, depression, and other factors. Effective and sustainable interventions are necessary since diabetes in older adults is associated with high mortality, reduced functional status, and increased vulnerability to cardiovascular complications (Asiimwe et al., 2020). Interventions should be tailored to the patient’s needs.
Several characteristics may impact socioeconomic/sociopolitical factors. For instance, high rates of diabetes affect people’s productivity hence lowering their income. Unproductive people are also unemployable, which may increase unemployment rates. From a sociopolitical dimension, type 2 diabetes increases healthcare costs by about $9,601 per person annually (American Diabetes Association, 2023). Such cost-related outcomes necessitate policies and advocacy initiatives at the community and national levels to protect populations. To achieve this goal, healthcare organizations should partner with government and community-based stakeholders to initiate sustainable health promotion programs.
These factors can influence patient outcomes and the financial viability of the organization in the community in multiple ways. When people understand the risks of type 2 diabetes, they will likely embrace programs that reduce the rates. For instance, they would be more positive to tailored messaging via telehealth, which is associated with diabetes medication adherence and improved hemoglobin levels (Dobson et al., 2020; Nelson et al., 2021). Advocacy, policies, and partnership can help the organization sustain its community programs due to a communal approach to a health problem. Such an approach reduces spending hence improving financial viability. Community-based approaches align with the norms of the U.S. healthcare system since they ensure health problems are addressed at the local levels. They further ensure that patients and populations understand their health problems, participate actively in health programs and that interventions are tailored to address patients’ needs.

Sample Answer 2 for DNP 840 Post your PICOT and expand in detail on your selected patient population
For diabetic patients in a primary care clinic, does implementation of American Diabetes Association Professional Practice Committee American Diabetes Associations Facilitating Behavior Change and Well-being to Improve Health Outcomes: Standards of Medical Care in Diabetes—2022 guidelines: Medical Nutrition Therapy Interventions impact HbA1C compared to current practice over 12 weeks?
My patient population is mostly lower socioeconomic individuals with little or no health insurance coverage (Edupuganti et al., 2019). A large majority are immigrants with questionable citizenship status. For example, I have Sudanese patients with limited English, and caring for them is sometimes challenging due to communication barriers. I and others use translation applications, but paid translation services are outside our budget. This clinic is a free service funded by donations, grants, and Medicaid or Medicare (Centers for Disease Control and Prevention, 2022). I have volunteered here for a few years and am still waiting to see a private insurance carrier. Many patients live in multigenerational homes whose income falls well below the poverty guidelines. Their food choices are limited as the clinic is in a desert. Hence, healthy, nutrient-dense foods are scarce. In addition, health literacy, educational levels, and access to transportation are limited. Therefore, if we reflect on Maslow’s needs theory, we realize that this population struggles with basic needs; and try as I may, those that can vote focus on essentials such as food and utilities. Unfortunately, their lack of participation in voting may reduce their probability of political change. Hence their circumstances are social determinants affect increasing their risk for adverse outcomes.
Sadly, this North Texas clinic resembles many faith-based outreach programs nationwide. Its multiracial and ethnically diverse community falls within the marginalized groups discussed in previous courses (Centers for Disease Control and Prevention, 2022). The health disparities result in more significant rates of diabetes, obesity, heart disease, and hypertension. Therefore the life expectancy for patients in the clinic is much lower. These factors will ultimately affect diabetic outcomes. What is clear is that we must tailor care to reflect the individual. The MNT process has several components, many encouraging a culturally specific diet. The MNT program also suggests including a dietician, education, and support. Incorporating these factors may assist with compliance and increase positive patient outcomes.

DNP 840 TOPIC 3 DQ 1 Sample Answer
Addressing healthcare disparities is a goal of medical organizations worldwide; in doing so, it positively changes the health delivery system at the micro, meso, and macro levels. Unfortunately, marginalized communities have poorer health outcomes; therefore, adding processes to improve well-being and promote health is critical (National Center for State, Tribal, Local, and Territorial Public Health Infrastructure and Workforce, 2022). Hence, the University of Minnesota’s Medical School Duluth Campus updated medical curriculums to provide a more inclusive program that addresses the needs of the Native American population (Weiner, 2017).
Native American health disparities are immense; this population has higher rates of chronic diseases such as heart disease, substance abuse, hypertension, and cancer (Weiner, 2017). In addition, historical atrocities such as forced sterilization created generational distrust; therefore, to offset this idealism, the addition of a cultural curriculum changed the healthcare landscape. Therefore the macro alteration in the delivery system can be observed through governmental organizations such as the Center for Disease Control and Prevention, whose Tribal Health division develops and supports public health initiatives to provide resources relevant to the American Native community (National Center for State, Tribal, Local, and Territorial Public Health Infrastructure and Workforce, 2022). For example, the meso healthcare delivery system was affected as ”the University of Minnesota Medical School, Duluth campus has since been approached by several medical schools seeking her input (Weiner, 2017)..” These subtle changes undoubtedly have influenced health care delivery at the micro level as patients in these marginalized areas are provided with culturally sensitive care that incorporates community health workers to assist with care delivery; ultimately, micro portions of the delivery system are impacted (National Center for State, Tribal, Local, and Territorial Public Health Infrastructure and Workforce, 2022). As you can see this innovative model of care is and Accountable Care Organization. Native Americans on reservations often qualify for Medicare or Medicaid services; providers then organize and coordinate, streamlining services to improve outcomes and decrease service duplication.
