Shopping online and getting it delivered to your house has been in market from a very long time, however, it was not until the pandemic when Instacart came into action and helped out so many families regardless of how risky the situation was for the young workers. Since then, Instacart has been a luxury, which many use however most of the people have returned to getting out and doing their own groceries. When we think about the aging adults, everything can wait except food. Food is something we intake every single day and there is no way out. Having the ease to get groceries delivered to your own home for the aging adults is an emerging trend that has made the lives of older adults a lot easier. Carrying and lifting heavy objects from the car to their homes is a difficult task for the older people, by having someone else do that and help you deliver it inside your doorstep is a big step towards advancement and improvement.
Having said that, Instacart is advancing in their services and aiming to provide more for the older population. Instacart is targeting food insecurities and aiming to allow all families and the elderly to have access to complete nutritional foods. Instacart is working on a senior program in which they plan to start addressing some of the major barriers that seniors are facing in accessing healthy food. They plan to partner with Alignment health, a Medicare Advantage company, and payment technology firm InComm Payments, to work towards this goal beginning this year. Starting in 2024, Instacart will allow its senior customers to use their supplemental health benefit cards to pay for its grocery fee services. Benefits will extend to Medicare Advantage enrollees, Medicaid recipients, and seniors participating in other eligible programs. Qualifying seniors will be able to use money from an applicable plan to pay for their groceries, over-the-counter medications, and wellness essentials, along with delivery, through Instacart. The only thing that can be seen from this emerging trend is goodness and ease for the aging population. In my opinion I do not see any major downside to this because everything is being accessed through benefits, which they were receiving previously. Even if new benefits are claimed the government will fund all of these. As a service provider, it would be beneficial for me to see seniors getting proper nutritious food and not struggle to afford the basic needs of life. Getting food and medication at your fingertips will make life so much easier and all of this is nothing but rewarding to see as a caregiver and a service provider.

According to Moy et al.(2023), there are three core Accountable Care Organizations principles:
1. Provider-led organizations with a strong base of primary care that is accountable for the quality and per capita costs
2. Payments linked to improvement in quality and reduced costs
3. Reliable and increasingly sophisticated measurement of performance to support improvement and provide confidence care is improved, and cost savings occur.
According to Colla et al. (2016), ACO groups’ focus is on those patients who are vulnerable, with complicated comorbid serious medical conditions. The ACO contracts hold the individual groups financially responsible for the quality and cost of the care delivered to Medicare patients (Colla et al., 2016). The risk and quality of care is now, such, that the providers are responsible.
In a cohort study of Medicare patients and a subgroup of vulnerable Medicare patients, it was determined that there was a $34 decrease in spending per patient per quarter for the Medicare populations and a $114 decrease in caring for the vulnerable Medicare population ( that is, those who have serious medical conditions) (Colla et al., 2016). While this is only a modest reduction in Medicare spending, it also reflects the reduced use of hospitals and emergency rooms as these patients are receiving quality care that is keep them out of the acute care circuit.
The Affordable Care Act’s purpose is to provide all Americans with access to affordable healthcare insurance. The further reform of this Act was the establishment of the Accountable Care Organization (Moy et al., 2023). The goal is to reduce spending and become more cost-efficient for the patients.

I enjoyed your discussion post this week on the advantages of InstaCart for the older adult. I had no idea that there were motions in this direction to enable the older adult to order their groceries online with the partnering of particular insurance companies. Being able to use their supplemental food assistance cards promotes a boon to hopefully developing nutritious eating habits. Instacart has announced that they will accept SNAP debit cards to buy groceries in all 50 states, as well as Washington, D.C. (Wilson, 2023).
Though some might feel that Medicare should be covering this service, and not a supplemental insurance company, there is a valid stance on that topic. The design of Medicare itself is to offer assistance in paying for medical needs (Moody, 2021). The argument may ensue that food, is indeed, a medical need for survival. The policy of Medicare is to cover expenses that are strictly medical in nature, and are necessary to preserve life and treat illness (Moody, 2021). The way around it is to utilize the Advantage plans, Alignment Health, and other accepted additional plans.
As you mentioned, the convenience for the delivery of the groceries, spares the older adult from handling the large parcels and bringing them into the home. Other advantages include the convenience and time saved to do other things while someone else is shopping for your needs. Maintaining independence is a top priority for many older adults, and this service provides shopping and delivery without having to impose on friends or neighbors. Sometimes, when someone offers to do the groceries, one may be prone to cut back on the list, so as not to be a bother, and subsequently go without something that would please them. Ordering the day before it is needed assures for fresh produce and dairy products; one needn’t “ stock up,” but can order anytime. Lastly, a trip to the grocery store can increase exposure to the various flu and upper respiratory infections, not to mention the struggle to find a parking space, walking in the inclement weather, or even risking a fall.
