As an Operating Room Nurse who precepts both new grad nurses and nurses new to the OR but who come from a different specialty, an area of concern I have identified is a lack of basic clinical skills that are important in a variety of specialty settings, including the OR. For example, as a nurse who first worked in med-surg before finding my home in the OR, I arrived knowing how to hang fluids, how to insert a foley and an IV and how to position patients for safety. However, when I am precepting new grads who come straight to the OR, I am finding that they lack these very basic skills and have only practiced them on mannequins in school.
A nursing theory that can be applied to address the issue of limited skills in new OR Nurses is Patricia Benner’s Novice to Expert Theory. This theory suggests that nurses progress through the development of skills and knowledge in five levels of proficiency: novice, advanced beginner, competent, proficient, and expert. The way to acquire skills and move through the levels is with continued education, experience, and time (Murray et al., 2019)
Using this theory we can assess the new nurse’s learning style, their self-evaluation of skills and taylor their learning in the operating room environment to the individual and their progression through the five levels of proficiency. There’ll always be a time in a nurse’s professional life when nurses encounter older patients and provide care, no matter where we practice our profession. My specialty area is geriatrics, also known as gerontological nursing, it is a branch of nursing that focuses on the providing care and hospitalization of the elderly. Caring for the elderly poses several difficulties for healthcare professionals. According to Adibelli (2016) that inadequate physical conditions and technical equipment in hospitals, care problems caused by patients’ physical restrictions, administrative difficulties, communication problems with the patient or family caregiver, and difficulties arising from a lack of knowledge, skills, and lack of experience in geriatric patient care are among the challenges faced by nurses in geriatrics. However, in my experience in this specialized field, a nurse’s role is critical in promoting active aging, and one area that I believe in that needs improvement is where geriatric patients are struggling with self-care and loss of independence. Loss of independence may be a problem for geriatric patients. While some elderly patients maintain a high level of independence, others increasingly require assistance with activities of daily living such as bathing, eating, and dressing. As elderly patients cope with their diminished abilities, they may experience negative emotions such as anger, shame, or sadness.
The nursing Need Theory, developed by Virginia Henderson, is the best theory to apply in this kind of situation. The theory focuses on a nurse’s role in helping patients maintain their health, recover from injuries, or die peacefully. Henderson’s theory emphasized the importance of nurses focusing on the patient rather than the healthcare providers (Petiprin, 2020). Furthermore, “the unique function of the nurse is to assist the individual, sick or well, in executing those activities contributing to health or recovery (or to peaceful death) that he would perform unaided if he had the necessary strength, will, or knowledge,” Henderson said. And to do so in a way that will assist him in achieving independence as quickly as possible.” Nurses would devote their time to patients, assisting them in relearning how to care for themselves (Norwich University, 2017). One of the theories I discussed last week was Orem’s Self-care Deficit Theory. Orem’s theory promotes the idea of patient independence over self-care with the ultimate goal of overcoming human limitations to self-care (Current Nursing, 2020). By focusing on a person’s ability to perform self-care, maintain health and overall wellbeing, this empowers the individual to take responsibility for their health or the health of others (Shah, et. al, 2013). This theory can be highly individualized from patient to patient, or patient’s families, so the application of this theory can differ greatly based on population focus. For example, this theory would be applied differently between adult and pediatric populations based on age and developmental stages. In pediatrics, this theory would likely be heavily geared towards parent support and educations in performing cares. Additionally, this theory can great vary based on specific illness or disease state. For example, I work in a pediatric cardiovascular ICU where the patient’s defect, arrhythmia, or ailment greatly determines their self-care needs. A neonate with a congenital heart defect that is not yet fully repaired may have an oxygen requirement or tube feeding support. In this case, much more teaching would be geared towards the parent’s ability to use/manipulate these new medical devices. On this same unit, I may also take care of an adolescent patient who just had a mechanical valve replacement. Initially post op, this patient may require full or partial cares, and these deficits would change as the patient status improves and once again becomes independent. This same patient may also require teaching involving anticoagulant therapy; many adolescents have the physical and mental capacity to administer their own medications, so teaching and support would be given in this case.
Ultimately, how Orem’s Self-Care Deficit Theory is applied to a population focus is highly variable from situation to situation because deficits can be adaptable and dynamic in nature.
