Re: Topic 1 DQ 1
For uninsured and Medi-Cal patients, does the use of physician led triage in the emergency department compared to nurse led triage, lead to shorter length of stay for non-acute visits after six months?
Physician In Triage (PIT) programs exhibit some benefits as they do reduce time from arrival to all results resulted (Franklin et al., 2021). Providers in triage allow for a quick disposition Although it does not shorten the overall length of stay in the Ed (Franklin et al., 2021). This study supports my PICOT statement as the average length of stay is for all patients of the ED whether they are discharged or admitted. Studies have shown that boarding hours contribute to extended length of stay. With PIT programs non acute patients will have a shorter length of stay as providers are able to move towards a disposition in a shorter amount of time.
There are some gaps as it relates to the literature review as the studies do not specifically state that provider lead triages will shorten length of stay for an uninsured patients, although the studies do imply that quicker dispositions will lead to quicker discharges which supports the need of PIT for uninsured non- acute ED visits. A study that analyzed an ED’s visits from July 2020 to July 2021 showed that 61.4% of 30,737 ED visits were non-urgent (Alnasser et al., 2023). If extended length of stay is attributed to increased boarding hours, it would be beneficial for PIT programs to be implemented in the fast-track area of the ED.

Sample Answer 2 for NUR 590 Based on the PICOT you developed for NUR-550, summarize the intervention you are proposing
In my current role, I focus on recruiting, retaining, mentoring, and converting nursing students into nurse residents. For the purposes of this discussion, I have developed a new PICOT statement and intervention relevant to my current population and setting.
New PICOT Statement:
- P (Population): Student nurse techs in a hospital setting.
- I (Intervention): Implementation of a mentor program where student nurse techs are paired with experienced RNs in their desired specialty.
- C (Comparison): No formal mentorship program.
- O (Outcome): Increased engagement and retention of student nurse techs.
- T (Timeframe): Six months.
PICOT Question: In student nurse techs (P), does the implementation of a mentor program where they are paired with experienced RNs in their desired specialty (I), compared to no formal mentorship program (C), lead to increased engagement and retention (O) over six months (T)?
Proposed Intervention: I am proposing the implementation of a mentor program designed to increase engagement among student nurse techs. Each student nurse tech will be paired with an experienced RN who works in the specialty they aspire to enter. The mentor will provide guidance, support, and professional development opportunities. This pairing will foster a supportive learning environment, help students gain practical insights, and build confidence as they transition from student nurses to nurse residents.
Supporting the Population of Focus: This intervention supports student nurse techs by providing them with a structured mentorship program that aligns with their career aspirations. It aims to enhance their engagement and commitment to the nursing profession. In my setting, which includes various hiring events, colleges, and hospital divisions, this program will help identify and nurture talent early, ensuring that students are well-prepared and motivated to enter the workforce as competent nurse residents (Cheyroux et al., 2023).
Role and Justification: As a regional nursing development specialist, my role involves recruiting, retaining, mentoring, and converting nursing students into nurse residents. The problem of student nurse engagement and retention is amenable to a research-based intervention because mentoring has been shown to positively impact professional development and job satisfaction (Bramble et al., 2018). By implementing a mentor program, we can address these issues in a systematic and evidence-based manner.
Conclusion: The proposed mentor program aims to increase engagement and retention among student nurse techs by providing them with dedicated support and mentorship from experienced RNs. This intervention aligns with my role and supports the organizational goal of developing a competent and committed nursing workforce.
Old PICOT Statement (for reference): In neonates requiring invasive ventilation (P), does having a combination of two RTs or RNs at the bedside for all hands-on care (I), when compared to just one licensed caregiver (C), lead to a decrease in unplanned extubations (O) over the next three months (T)?

NUR-590 Topic 1 DQ 2 Sample Answer
Having a spirit of inquiry refers to people who have the drive to find to find the best evidence based practice solutions to help drive decision making (Bell, 2021). Having members of your team that have this drive is a must in order to encourage a culture that is evidence based practice based. As an advanced practice nurse, we normally take on a leadership role which requires for us to lead by example. This is where it would benefit the team for us to lead with the spirit of inquiry in the interventions that we put into place. Nurse practitioners are another example of an advanced practice nurse. These type of advanced practice nurses have different roles than the type of nurse we are currently going to school for. In the 2022 edition of Nursing Scope and Standards of Practice defined one of their roles as a champion for scientific inquiry (Brunt, 2023). This ideal help them to drive their practice in the direct care of patients, which is something that we wont always encounter in the work that we do. Regardless of the specifics of our job description, as advanced care nurses leading with evidence based practice interventions will give us the best outcomes.
