The Affordable Care Act’s (ACA) Medicaid expansion became optional across the United States effective January 1, 2014. The US Supreme Court decision was to make the Medicaid expansion component optional to states to cover everyone with an income of up to 138% of the federal poverty level (Richardson & Lilliard, 2019). Not all states opted to expand Medicaid eligibility, but twenty-eight states and the District of Columbia did. Most other states, commonly under political (Republican) control, opted out (Singer & Rozier, 2020). Not all states decided on the Medicaid expansion due to considerable bipartisan approval and concerns about insurance coverage and access (Perez, 2018). After the ACA and Medicaid eligibility, within the first two years, there was a 17.7 decline in adults without health insurance, with states that expanded Medicaid having the highest decline (Parzuchowski et al., 2023). An estimated 20 million people, as of 2017, gained health insurance coverage as a result of the ACA. Although statistics show a reduction in uninsured individuals and expanded Medicaid numbers, the debate is still occurring among states about the long-term benefits of the ACA and what the future holds regarding affordable healthcare for individuals without insurance.
