Examples of these updates include alerts of patient movement that are generated from sensors that can recognize when the client try to get up or bed height and side rail position. However, time and human ingenuity will fix these concerns.

The biggest change and trend that I most recently experienced is the constant use of telehealth/e-visits, Skype, Zoom, and Microsoft Teams due to the COVID-19 pandemic. Telehealth is the use of technology to deliver health care, health information, or health education at a distance (McGonigle & Mastrian, 2018). A few ways telehealth is used in the clinical setting are sharing images amongst disciplines for assessment and diagnosis, transmit data, provide disease prevention information and promote good health while using real time video and phone services (McGonigle & Mastrian, 2018).
The use of all of these new trends have definitely become the “new normal”. Meetings are now virtual to permit social distancing and many visits and consultations are through the telephone. A challenge of the virtual meetings could be employee engagement and employees actually paying attention to all being said throughout the meetings. As for telehealth services and e-visits, I worry of the challenge that patients can be wrongly diagnosed and improperly assessed during their verbal only visits. As professionals we rely on our assessment skills to care for patients. Although our care has a great amount to do with what the patients tell us, we also use our skills of visually examining and touching patients to make the most accurate diagnosis and provide the best care for symptoms. Patient bills are also the same as an in-person visit, for a phone call that is sometimes only five minutes.
The biggest benefit of this virtual trend is safety amongst health care professionals and patients. Also, it could benefit providers to limit non-emergent in person visits to avoid possible exposures. As far as possible risks, patient information can possibly be compromised, and higher security must be used so information remains protected. Since private in-person rooms are not available it is still recommended by the Department of Health and Human Services to use private locations and patient should not receive services in public or semi-public settings (Jacobson, 2020). Throughout the pandemic there were also many cases of hacking into Zoom and Skype meetings. Providers must do all they can to avoid these data breeches to protect patient information.
Telehealth and e-visits do show advancements for nursing practice as it opens more career opportunity. “Telehealth eases the impact of the nursing shortage because it provides easier access to professionals for patients” (Teladoc Health, n.d. para # 5.) According to the American Telemedicine Association, within five years, 50% of healthcare services might be provided by telehealth services (Teladoc Health, n.d.). Along with career advancements, this will allow nurses to increase their assessment skills and critical thinking with the need of accurately finding treatment for patients. With time and growth in this particular area, I think there will be growth in this area and providers will accurately provide care for patients. With continued time, I also think there will be improvements to data management and protecting patient information.

One idea I would like to add to your discussion about patients being unable to access telemedicine services is that many people also lack adequate broadband coverage to use these services. Especially in my home of rural West Virginia, many people cannot use telemedicine because of no or inadequate internet access. According to Sharma & Patten (2022), “approximately 162.8 million Americans have no access to the internet” at broadband speeds meaning that these “digital health care service delivery would likely fail in the communities where these standards are not met” (p. 150). In agreement with your belief that telemedicine positively impacts healthcare, Akareem et al. (2021) found that those who engaged with technologies such as a patient portal positively impacted their well-being outcomes. More specifically, Carini et al. (2021) found improved health outcomes and treatment adherence in patients that used the portal. Patients who used the portal were significantly more likely to control their diabetes and blood pressure than those who did not (Carini et al., 2021). Healthcare technology advancements such as electronic health records and telemedicine help shape healthcare to improve patient outcomes and efficiencies.

While the pandemic forever changed our economy, our society, and how healthcare systems function, the advantageous use of telehealth technology was catapulted at a breathtaking pace, transforming the delivery of healthcare in unprecedented swiftness. There was no time to think, only time to change and accomplish. Telehealth refers to the wide range of activities that are used to deliver care from a distance. The use of telehealth and virtual care technology allows caregivers to overcome physical barriers, such as geographical boundaries, and still provide the much-needed care patients deserve (Wosik et al., 2020).
During the pandemic and now post-pandemic, it has forced individuals, communities, and healthcare systems to review what and how we operate. It forced us to review what was possible and what was needed to adapt the current models of care to meet the rising demands that the current situation demanded. Many countries and healthcare systems have shifted towards telephone and video consultations. We have sent patients home with technological devices such as pulse oximeters and other healthcare monitoring devices, as well as instructions on self-management to minimize the load that healthcare systems were experiencing. Technologies to support telehealth are increasing and include wearable devices, smartphones, and the design of (smart) homes. Smart homes can be equipped with environmental and personal sensors that are interconnected using the Internet of Things. Devices within smart homes can monitor a patient’s health and if needed, send messages to providers if emergency situations arise (Blandford et al., 2020).
As with anything related to technology and the transmission of healthcare-related information, patients and professionals must trust digital systems to keep health information private and secure, in order to realize the full potential of telehealth. The most current updated data security and encryption systems should and must be deployed to protect the privacy of our patients and to ensure patient trust. While healthcare systems need to put in place the most current security protection systems, patients have the responsibility to make themselves aware of the privacy choices they are making and to ensure that they don’t disclose sensitive personal data over unsecured systems. At the end of the day, telehealth comes with inherent risks including exacerbating the digital divide among populations, inadequate software engineering, and security breaches. Moving forward, telehealth platforms must be secure, trustworthy, and flexible enough to accommodate the ever-changing regulatory, professional, and healthcare systems requirements (Blandford et al., 2020).
In the coming decades, a significant increase in information technology, such as telehealth, is expected. We are currently experiencing a nursing and healthcare worker shortage across the nation and across the globe. In my current hospital, shortages are seen in environmental service (EVS) members, certified assistive personnel (CAPs), nurses, and all the way up to specialized physicians (ie. Neuro-Interventionalist, Interventional Cardiologists, and Electrophysiology Physicians). These growing trends will be the driving force toward increased telehealth and technology use. Chronic disease, growing populations, and the need to reach more remote populations will make the growth of telehealth that much more important (McGonigle & Mastrian, 2021, p. 407).

Emerging technologies have made it possible for people to get healthcare on the spot. They could be at home, out for a jog or in their favorite restaurant tracking their calories to stay on top of their body’s health. With access to an internet connection a wide array of software, websites or information concerning health awaits the modern man. Access to the internet is not a right however useful as it may be to people. It is not handed to you and neither is it free to connect to. This fact alone qualifies the issue at hand to be a Social Determinant of Health in my opinion. Cybernetic technologies associated with an Internet connection can significantly improve quality of healthcare to patients. Healthcare workers have started embracing all the aspects of technology such as telemedicine, secure online portals, online patient portals, EMR- electronic medical records and immersive technology. (Sharma, 2023).
Patient portals are websites that help patients keep track of their health. Use of patient portals can be traced back to the 2009 Health Information Technology for Economic and Clinical Health (HITECH) Act and the Medicare and Medicaid EHR Incentive Programs, now known as the Promoting Interoperability Programs. (Carol, 2022) They keep a record of prescriptions, health care provider visits, billing, test results, among other things. (Talking Health Tech, 2021). These portals are aimed at improving quality of healthcare provide to patients while enhancing communication between physicians and their patients. (Sarkar, 2023). This communication also helps care providers to provides better care to their patients as it guides them on scheduling based on severity of the patient’s disease.
Patients are able to access their medical information without having to go to the clinic or hospital. This saves both time and money when commuting is factored in. There are a number of patient portals available therefore patients are spoilt for choice and can choose one that suits them best. Patient portals provide accurate and reliable information because chances human error is minimized. Being able to receive messages from patients allows care givers to correctly interpret what their patients need so that they are well informed and can make the appropriate decisions. (Sarkar, 2023).
Use of patient portals to care for patients faces some challenges however. Internet access and a computer or a smartphone are needed to use patient portals. According to Dempsey (2023), affordable broadband is particularly scarce in rural and/or low-income areas. Environment, a social determinant of health thus deters patients living in rural areas from using patient portals. More so, patients from underprivileged communities may have a hard time affording these tools. High speed internet is moderately getting more and more expensive. Efforts should be made by governments and Internet Service Providers to increase Internet coverage and accessibility while lowering costs for subscriptions. This will definitely solve the issue of inequality in receiving better quality healthcare through patient portals. Lack of time could also explain why many people don’t use patient portals despite their many advantages. It is important for health organizations to reach out to such individuals to educate them on the benefits of signing up to patient portals.
In conclusion, nurses and other healthcare workers should be at the forefront in advocating for the need to use patient portals. It helps them be equipped better to deal with patients therefore offering high quality healthcare to their many patients. Patient portals are thus way too important to be ignored and it would be unfortunate for patients who care for their health not to use them. It is conclusively evident that digital inclusion and broad band access should be considered to be major social determinants of health.
